Minimum Qualifications:

• High school/GED diploma
• HHA/CNA certificate preferred but not required

Job Duties:

• Perform floor ambulation for residents as prescribed by the rehab department/MD
• Administer AAROM/PROM for residents as indicated
• Clean and sanitize surfaces and equipment as part of the functional maintenance program
• Transfer residents in/out of the wheelchair as part of functional maintenance
• Ensure proper application/removal of splints/braces by nursing staff as per MD order
• any inconsistencies, functional changes, or skin integrity issues to the nursing manager and/or rehab director
• Document all FAP distance and level of assistance daily in EMR
• Conduct group wellness exercises with residents on the units as directed by Rehab therapy staff
• Other duties as assigned by the Rehab director
