Eastern Virginia Medical School Hiring for Maternal Child Health Educator at Norfolk, VA Full Time

Overview:
The Maternal Child Health (MCH) Healthy Start Educator position provides in home comprehensive assessment, educational and nursing services to high-risk mothers, children and their families in their home and community with the goal of reducing maternal and infant mortality and improving maternal and child health outcomes as part of the EVMS Healthy Start Loving Steps program. This is a part time position.

Responsibilities:
Establishes a relationship with families by providing regular contact through home visits and phone calls.

Serve as the staff lead to provide guidance on medium and high-risk women identified through the screening tool with risk assessment as having chronic disease (i.e. diabetes, hypertension, etc.), behavioral or mental health issues, and significant reproductive health risks;

Provide nursing assessment and education through home visits and group education to enrolled clients on their caseload and to clients on other workers’ caseloads in a consulting capacity as appropriate.

Implement all components of the program with clients as outlined in the program manual in addition to nursing specific assessments, education and activities defined by the Loving Steps (LS) program;

Communicates with MD, family and interdisciplinary care providers to clarify care and increase awareness of significant changes in condition in a timely manner.

Link or assist the Loving Steps team to link, clients with identified risks to appropriate service in the community.

Provide training and education to the Loving Steps staff team on issues related to health, such as reproductive methods, injury prevention, mental/behavioral health and developmental screening.

Educate and guide team members on when and how to seek nurse consultation related to a client’s case.

Lead or consult on health related group education for clients

Transports families to and from doctor appointments, social services agencies, and program related events in EVMS provided vehicles

Performs other duties as assigned.

Qualifications:
RN diploma. Must be a licensed RN in Virginia. Five (5) years’ of experience and knowledge of community based public health approaches and maternal and child health.

VA driver’s license required. Must be insurable by EVMS’ insurance carrier.

It is the policy of Eastern Virginia Medical School, (EVMS) that all EVMS owned vehicles be will be operated by authorized drivers for EVMS. New employees whose positions include driving an EVMS vehicle as one of their job requirements, or a current EVMS employee whose position or job requirements change to include driving an EVMS vehicle, will be required to become an EVMS authorized driver.
