Key Responsibilities

• They assist individuals in determining eligibility for social programs and financial assistance while connecting them with various special funds and agencies within the community.
• Under the supervision of a licensed clinical social worker, they provide crisis intervention and mental health assessments to patients and families with psychosocial needs.
• The role involves coordinating and facilitating the development of comprehensive discharge plans for high-risk patient populations.
• Collaborates with an interdisciplinary team, including Physicians, Case Managers, Staff Nurses, and other care team members, to address psychosocial issues and ensure appropriate treatment across the continuum of care for at-risk individuals.
