a. Responsible for providing the PAT service to enrolled families in accordance with the standards defined by the OSDH statement of work.
b. Responsible for providing the PAT service to enrolled families in accordance with the standards defined by PAT’s affiliate status.
c. Responsible for providing service consistent with the Strengthening Families Protective Factors.
d. Will assume a whole person approach and will screen for medical care needs, link to medical providers, and provide health promotion education consistent with their level of training and experience.
e. Provide advocacy, linkage, and referral services on behalf of families enrolled in PAT.
f. Will provide psychiatric rehabilitation services as appropriate to families enrolled in PAT.
g. Responsible for conducting Group Connections monthly.
h. Responsible for maintaining a minimum family capacity of 85%.
i. Responsible to develop and maintain referral sources for PAT.
j. Responsible for developing and maintaining referral sources for PAT.
k. Responsible for submitting all program evaluation data in a timely manner.
l. Participate in at least 4 hours of reflective supervision monthly and at least 4 hours of staff meetings monthly.
m. Maintain the clinical record system in accordance with OSDH, ODMHSAS, OHCA, CARF and CREOKS rules and regulations.
n. Responsible for attending mandatory training and maintaining their PE certification.
o. Responsible to read and be knowledgeable of all CREOKS policies and procedures.
p. All other duties as assigned.

Qualifications/Competencies Required
· Must have at least a high school diploma or GED and a minimum of two years’ work experience with young children and/or parents OR to have a two-year degree or 60 college hours in early childhood or a related field.
· Must be able to model PE skills and attitudes (strength-based, non-judgmental, etc.)
· Must obtain and maintain Foundational and Foundational2 Subscriptions
· Must complete FSP training through the ODMHSAS
· Must be able to establish and maintain strong relationships with other agencies
· Must be able to conduct public presentations
· Must be able to work as part of a team.
· Must abide by ethical policies and practices for both CREOKS and State oversight agencies and boards
· Must understand and abide by all legal aspects of clinical practices
· Must be able to provide culturally competent services
· Must be able to assess and coordinate the work of clinical and support staff
· Must have a conceptual grasp of community-based mental health and substance abuse services
· Core competencies in relation to the Strengthening Families Protective Factors.
