As a Social Worker/TR Assessment Assistant, you’ll work alongside our Director of Social Work and Therapeutic Recreation and other core members of our team. Your role will have a focus on residents with cognitive loss at all levels. You will be providing high-quality assessments and plans to maximize resident function.

As the Social Worker and Therapeutic Rec Assessment Assistant at Country Manor, you’ll:

• Interviews residents with cognitive loss and completes assessments
• Provides all aspects of medical social work on the LTC Memory Loss u
• Develops short term care plan based on assessment findings in the area of TR.
• Document finding(s) and communicates with TR staff and other disciplines on the appropriate TR care plan for the individual.
• Completes TR progress notes and ongoing monitoring of progress.
• Provide written documentation for Social Services prior to care conference.
• Interviews residents with cognitive loss and completes Social Services assessments including Psychosocial/Social Service Assessment, BIMS, and PHQ-9 interviews.
• Develops care plans for residents in the area of mood/behavior, psychosocial needs and daily routines
• Provide therapeutic recreation programs on a fill-in basis. Document attendance, transport residents and facilitate participation and program planning.
• Provides 1:1 visit for residents that are limited in their mobility, desire or overall condition to attend out of your events. Documents as needed.
• Provides fill-in medical social services as directed

Qualifications Required:

• Bachelor’s degree in social work
• Licensed in the State of MN or actively pursuing license in Social Work

Candidate must possess a current MN Social Worker license or actively pursuing license. This is a full-time position, primarily day shift but the possibility of varied hours.
