If you join Cosán Group, you will be part of our amazing team of Care Coordinators. You will build a rapport with your patients through monthly calls as you support them through services such as Chronic Care Management (CCM), Behavioral Health Integration (BHI), Principal Care Management (PCM) and Remote Physiological Monitoring (RPM). Furthermore, you will empower patients to meet their goals, guide them in their healthcare journey and advocate for their needs.
In this role you will act as an extension of the provider to help close the gaps in your patient’s care.

What to expect your first 2 weeks: Your first two weeks will be an orientation led by our expert Clinical Trainers where you will learn the ins and outs of Cosán as well as work through our proprietary platform. What to expect your first 90 days: Out of orientation you will be introduced to the team you’ll be working alongside. Your Team Lead and Clinical Manager will teach you account specifics and guide you through your first steps of making patient calls. You will also have scheduled check-ins with your Team Lead & Manager during the first 4 weeks out of orientation. Trainers will always be available for added support as well throughout your Cosán journey!
