Job Description

As a Nurse Care Manager at Compass Health Network, you will be an integral part of a person-centered integrated care team, delivering individual and population-based health interventions to individuals enrolled in the Health Care Home program. You will play a crucial role in integrating physical health care into behavioral health settings to ensure quality and compassionate care in our service areas.

Responsibilities

• Manage a case load of up to 200 Healthcare Home clients, completing monthly interventions and care in compliance with DMH regulations
• Conduct health screenings, identify risk levels, establish care plans, and provide recommendations
• Collaborate with clients to set physical health goals, promote healthy lifestyles, and encourage preventive measures
• Facilitate referrals to primary care physicians or specialists, educate clients on the importance of regular screenings, and promote appropriate healthcare usage
• Perform Metabolic Syndrome Screenings and Medication Reconciliations following agency guidelines
• Provide education to clients and caregivers on physical health conditions, treatments, screenings, and self-management
• Offer consultation and education to staff on health conditions and collaborate with healthcare providers and social service agencies
• Attend team meetings, agency meetings, and relevant trainings to provide holistic services

Requirements

• High School/GED required
• Practical Nursing or Associate of Nursing degree required; Bachelor’s degree in nursing preferred
• Licensed Practical Nurse (LPN) required; Registered Nurse (RN) preferred
• Proficiency in Microsoft Office Suite and Electronic Medical Records preferred
