BASIC FUNCTION:
This position is for a highly responsible professional who provides parenting education, care coordination and enhanced services to pregnant women and children to age three in accordance with Healthy Start standards and guidelines.
SUPERVISORY ACCOUNTABILITY:
As support staff, this position will interact with nursing staff, management, physicians, external educational vendors, and community resources.
NATURE AND SCOPE:
The position requires daily communication and interaction with clients, team members (both inter and intra-departmental), management and medical staff, as well as frequent external communication, referrals, follow-up and collaboration with community resources/agencies.

Knowledge of purpose and services available through Healthy Start Coalition programs.
Thorough understanding of the principals of “Healthy Start Standards and Guidelines.”
Knowledge of federal and state laws, rules and regulations related to Healthy Start, MomCare, and other program related legislation.
Ability to communicate effectively.
Knowledge of community, social, and healthcare resources and Medicaid.
Ability to plan, organize and coordinate work activities.
Discretion and judgment required in order to handle sensitive and confidential situations.
Interpersonal skills requiring courtesy, tact, confidentiality, and empathy at all times, in order to communicate effectively and convey information to callers, visitors and coworkers.
Knowledge of and experience with computer hardware/software including, Microsoft Office software and Internet.
Bilingual in English and Spanish preferred.
Meets all competencies for Care Coordination Providers as identified in the “Healthy Start Standards and Guidelines – Chapter 4.”
CORE COMPETENCIES:
Create and maintain a positive image of the Indian River County Healthy Start Coalition, Parents as Teachers, Cleveland Clinic Indian River Hospital, and the Healthy Start Programs.
Provide parenting education, case management, and ongoing care coordination according to the risk appropriate criteria for all Healthy Start participants in accordance with the “Healthy Start Standards and Guidelines,” Parents as Teachers, and the local Indian River County Healthy Start Coalition criteria per the community needs assessment.
Complete an initial and annual family-centered assessment with each family.
Provide personal visits focused on parent-child interaction, development-centered parenting, and family well-being (including planning, delivering, and documenting the visit).
Use the Parents as Teachers foundational curriculum to share research-based information with families.
Provide and participate in group connections to give families the opportunity to form social connections with each other.
Complete an initial and annual developmental screening and health review for each enrolled child.
Connect families to resources that help them reach their goals and address their needs.
Maintain and submit all required family and program documentation in a timely manner according to guidelines.
Participate in continuous quality improvement.
Care coordination services should also include: providing education, instruction on filing grievances, assisting with referrals to community resources, explaining Coalition program services.
Coordinating care with all Healthy Start Coalition programs, WIC, and other community resources.
Complete Individualized Plans of Care (IPC) for Healthy Start participants as specified in the “Healthy Start Standards and Guidelines” and utilize Parents as Teachers or FSU curriculum, “Partners for Healthy Babies.”
Develop Family Support Planning for all participants in need of high-risk, ongoing care coordination based on the criteria of the leveling system in accordance with the “Healthy Start Standards and Guidelines.” Update Family Support Plan as per standards and guidelines.
Develop, monitor, and review goals with each family.
Accurate data entry of information into the Well Family System and Penelope as indicated.
Secure and protect client confidentiality and personal health information.
Prepare and maintain various reports and documents using computer applications when applicable with attention to detail.
Participate in required program trainings provided by the Indian River County Healthy Start Coalition, as well as staff meetings.
Perform other duties as assigned.
MINIMUM REQUIREMENTS:
Graduation from an accredited college or university with a Bachelor’s Degree in childhood education, social work, health, psychology or a related field recommended. Consideration will be given to candidates with a two (2) year degree, or 60 college hours in early childhood or a related field. Must have a minimum of two (2) years of previous supervised work experience with young children and/or parents.
Strong communication and interpersonal skills (e.g., nonjudgmental, objective, reflective, empathetic, patient, tactful).
Ability to establish rapport with families and empower them by building on their strengths.
Strong organizational and record keeping skills.
Ability to work independently and be self-motivated (often in the field).
Computer skills including web-browsing, e-mail, Internet, and word-processing.
Reliable transportation.
