The Care Coordinator assists participants in identifying and gaining access to medical, social, educational, and other services. The Care Coordinator encourages the use of community resources through referral to appropriate traditional and non-traditional providers, paid, unpaid, and natural support. Care Coordinators work in partnership with the youth and their family/caregiver/legal guardian and are responsible for assembling the child and family team for a Child and Family Team Meeting (CFTM).
The Care Coordinator must be emotionally stable, able to handle high stress, relate well to a wide variety of individuals, and be able to work as part of a team. The Care Coordinator must be able to follow, enforce and implement program descriptions as outlined in policy and procedure.
The Care Coordinator must be able to provide one-to-one interaction with a designated child/adolescent and their family; act as liaison and advocate for youth and families with the therapist and other team members or other agencies as needed, complete required paperwork, and perform other related duties as required. Care Coordinators must be flexible in working hours (available in the evenings after traditional work/school hours to avoid disruption of their day), be able to respond to crisis situations in an effective manner, and meet regularly with their supervisor.
This position provides care coordination to consumers/residents/members and the primary duties include but are not limited to the following:

Salary: 40,000 to 45,000 (depending on experience)

This position typically works 10 am -6 pm Monday through Friday with occasional evenings and Saturdays to attend community events. (Time may vary based on the needs for the program and the needs of the community) This position requires flexibility. The position also requires some light travel.
