Definition:

Care Coordinator I – oversees client care experience in all aspects. Maintain internal systems and processes to ensure the agency is in compliance with state, federal and local regulations. Care Coordinator I’s to the Director of Care.

Level of Responsibility:

Works under the supervision of the Director of Care.

Duties and Responsibilities:

• Complete new admissions for consumers, including the required paperwork.
• Obtain all needed paperwork for consumers to become active.
• Coordinate all other care in the home and document in the consumer record.
• Communicate with consumers on schedule and needs.
• Ensure all consumers have appropriate staff assigned to meet their needs.
• Perform supervisory visits to each consumer at least every three (3) months or more, if necessary, for:
• Evaluation of the client’s current and on-going condition.
• Clients’ satisfaction of care.
• Evaluation of the client’s current home condition.
• If care staff is present, evaluation of a performed care or service.
• Review of the care plan to gauge its effectiveness and ascertain if any changes are necessary.
• Problem resolution.
• Complete annual visits and all communication, paperwork, and follow up that is required.
• Process all PAR’s, verify authorization’s & communicate to appropriate staff on actions needed.
• Communication with Case Manager(s), physician(s) and other providers on the care plan as necessary to assure appropriate and effective care.
• Adjust schedules and care tasks as consumers needs change. Communicate to appropriate staff if any actions are needed.
• Complete change of condition re-assessments as needed.
• Update supervisor on all actions and priorities on a regular basis at a minimum weekly.
• Investigation, documentation and follow-up of complaints, occurrences, and critical incidents.
• Counsel staff on difficult cases and potentially dangerous situations.
• Provide discharging consumers with the needed information to continue care.
• Complete the internal steps to discharge a consumer.
• Document all interactions with clients and staff.
• Communicate with team members on processes to ensure internal processes are followed.
• Ensure compliance with all processes and actions are within agency standard.
• Attending all staff meetings as they are scheduled.
• Maintaining and responding to emails and all inter office communication.
• Perform employee reviews for appropriate relative and outside care staff for case load.

Minimum Qualifications:

• The Care Coordinator I shall be at least 21 years of age.
• Must have appropriate experience in Care Coordination of home care or closely related services.
• Must have successfully completed the Agency’s PCW Training Program and Agency Orientation.
• Must have advance knowledge of Excel and Word.
• Must have effective communication skills both verbal and written.

Knowledge, Skills, and Abilities:

• Knowledge of principles and procedures of home care attendant/caregiver and safety practices in the home care setting.
• Understanding of family interactions and aging process.
• Beginning knowledge of nutrition and food service.
• Beginning knowledge of the skills completed by the attendant/caregiver care.
• Ability to observe, and record client’s care and condition accurately.
• Ability to communicated effectively both orally and in writing.
• Ability to establish and maintain effective working relationships with management, clients and families and agency staff members.

Special Requirements:

• Valid, current driver’s license and a reliable automobile or access to a reliable automobile, public or other reliable transportation.
