CenterCare Home Health Hiring for Physical Therapist (PT) and PTA at Denver, CO Full Time

Castle Rock to South Denver area

Mileage reimbursement will be paid at $0.50/mile Full-Time and PRN

CenterCare Home Health is looking for a registered Physical Therapist for therapy services in patient homes. Full-time is preferred, however will consider candidates interested in PRN. They will provide initial and ongoing assessments based on the patient needs along establishing treatment plans . They will also be responsible for supervision of Physical Therapy Assistants, along with educating families and patients.

Responsibilities

• Identify patient’s needs and exhibit compassion, caring, and empathy to individual differences
• Recommend options, courses of action or modifications in selected treatments ensuring patient’s progress, safety and comfort
• Apply the latest research related to restoring function, reducing symptoms frequency, and preventing injury
• Prepares assessments or patients condition and periodic evaluations and summaries
• communicates with Physician on changes and obtaining additional orders
• writes clinical/progress notes on patients
• Participate in patient case conference
• Adhere to ethical and legal standards
• Ensure continued competence and updated field knowledge

Qualifications

• Must have graduated from a Physical Therapist curriculum that is accredited by the commission on Accreditation in Physical Therapy Education (CAPTE) of the American Physical Therapy Association (APTA), or its successor organizations
• Must be licensed in Colorado
• Must have at least one year of institutional/home care experience
• Ability to communicate and instruct effectively
• Must have a criminal background check
• must have current CPR certification

Job Types: Full-time, Part-time, PRN

Pay: $50.00 – $120.00 per hour
