Update s weekly for adherence, transition of care (TOC), and medication therapy management (MTM)

Maintenance for telephonic outreach: IVR and dialer Create weekly agenda for team meeting (gather and post notes)

Track and trend call volumes and outcomes of interventions provided

Support Director related initiatives which may require building member lists, phone trees, vendor oversight, etc.

Management and oversight of team members including goal tracking, timecard keeping, weekly assignments, ing, evaluating, and running 1:1/team meetings

Salary Range: The salary for this position is between $34000 to $36000 annually. Factors which may affect pay within this range may include geography/market, skills, education, experience, and other qualifications of the successful candidate.
