JOB DESCRIPTION: NOW HIRING PART TIME, PER DIEM, FULL TIME $75-$85 PER VISIT

Licensed Physical Therapy Assistant

JOB SUMMARY:

A Licensed Physical Therapy Assistant (LPTA) administers physical therapy to patients on in their place of residence. This is performed in accordance with physician orders and IDG plan of care under the direction and supervision of the Registered Physical Therapist (PT).

QUALIFICATIONS:

• Graduated from a physical therapist assistant (PTA) curriculum approved by the Commission on Accreditation in Physical Therapy Education of the American Physical Therapy Association; or if educated outside the U.S. or trained in the U.S. military, graduated from an education program determined to be substantially equivalent to a PTA entry level education in the U.S. by a credentials evaluation organization approved by the American Physical Therapy Association; and
• Passed a national examination for physical therapist assistants.

On or before December 31, 2009, meets one of the following:

• Is licensed, or otherwise regulated in the State in which practicing.
• In States where licensure or other regulations do not apply, graduated before December 31, 2009, from a 2-year college level program approved by the American Physical Therapy Association.

Before January 1, 2008, where licensure or other regulation does not apply, graduated from a 2-year college level program approved by the American Physical Therapy Association.

On or before December 31, 1977, was licensed or qualified as a PTA and has achieved a satisfactory grade on a proficiency examination conducted, approved or sponsored by the U.S. Public Health Service.

• Currently licensed in the state(s) in which practicing.
• Two (2) years experience, preferred.
• Acceptance of philosophy and goals of Hospice.

RESPONSIBILITIES:

• Understands and adheres to established Hospice policies and procedures.
• Provides physician and IDG prescribed physical therapy under the supervision of a PT.
• Participates with IDG and all other health care personnel in patient care planning.
• Performs all procedures as ordered by physician and IDG and according to the plan of care for therapy established by the PT.
• Consults with PT regarding change in treatment.
• Instructs patients and family members in home programs and activities of daily living.
• Participates in inservice programs and presents inservice programs as assigned.
• Participates in QAPI activities as assigned.
• Attends all patient care conferences as scheduled.
• Prepares IDG progress notes for each patient visit in a timely manner as per Hospice policy.

• Performs services planned, delegated and supervised by the PT.
• Participates in educating the patient and family.

WORKING ENVIRONMENT:

Works indoors in Hospice office and patient homes and travels to/from patient homes.

JOB RELATIONSHIP:

• Supervised by: Physical Therapist/Nursing Supervisor/Director

LIFTING REQUIREMENTS:

Ability to perform the following tasks if necessary:

• Ability to participate in physical activity.
• Ability to work for extended period of time while standing and being involved in physical activity.
• Heavy lifting.
• Ability to do extensive bending, lifting and standing on a regular basis.
