The Healthcare Services Coordinator (hereafter, Coordinator) plays a pivotal role at Adapt Integrated Health Care, providing comprehensive support and excellent customer service to our patients, patient family members, Adapt staff, and the public. The Coordinator represents Adapt’s mission and values and serves as an ambassador of Adapt’s patient centered No Wrong Door Approach , ensuring access to the most appropriate care regardless of a patient’s point of entry. The Coordinator is knowledgeable about Adapt programs and services and is responsible for greeting, directing, and proactively providing information and assistance to patients, families and visitors in accordance with Adapt Integrated Health Care policies and procedures. The Coordinator manages a variety of clerical and administrative tasks, including but not limited to patient registration, scheduling, data management, and duties necessary to ensure a seamless flow of information within the department.
