• Description:*
• Employee will be performing member outreach.
• Coaching members into healthy habits and gap closures. regarding coverage and provider visits.
• Assist with membership attribution
• Assist with provider and patient engagement
• Assist with solving for SDOH concerns
• Provide resources to members to improve health outcome
• Skills & Qualifications:*
• Bachelor’s Degree in Healthcare, Public Health, Nursing, psychology, Social Work, Health Administration, or related health field or equivalent work experience
• LPNs or Health Coach preferred
• Nurses must have an active compact license for TN,MS, or AR
• Clinical experience working in a physician’s office
• Previous clinical outbound call outreach; motivational interviewing
• Case Management/Utilization Management
• HEDIS
• Experience Level:*
• Intermediate Level
