THE TYPE OF PERSON WE’RE LOOKING FOR

We’re searching for someone who can meet the following qualifications:

• Master Social Worker license in the state of Texas OR a master’s degree in a related field
• 2+ years of CLASS case management experience
• 2+ years of experience providing services to children or adults with disabilities
• Driver’s license and proof of automobile insurance
• Knowledge of Microsoft Office products, including Word, Excel, and Outlook
• Ability to learn to use our client database efficiently and independently
• Ability to work independently with a minimum of supervision.
• Fluency in both Spanish and English is preferred but not required

What It’s Like To Be Our Care Coordinator

You will play a vital role in providing comprehensive case management services to individuals enrolled in the Community Living Assistance Support Services (CLASS) Medicaid Waiver program. Working under the direction of the Senior Director of Community Supports and the Vice President of Programs and Services, you will uphold the established program standards outlined in the CLASS Provider Manual. You will serve as an advocate for the clients assigned to your caseload, ensuring that their needs are met and that they receive the highest quality care. You will collaborate with the administrative assistant when appropriate and seek guidance and clarification from the Senior Director as needed. In this role, your longevity and expertise in the program elements will position you as a valuable resource and mentor within the team. You will have the opportunity to contribute to the training of CLASS Case Managers, further enhancing the overall knowledge and effectiveness of the program.
