Purpose: The person in this position will provide support to the Grace Hospital Liaisons.

Job Summary:

• Track referrals working with the team and Intake to ensure that all information is given to Intake so referral can be processed
• Meet with families to secure consents, explain the hospice benefit and ensure expectations are set
• Work with hospital Case Management and SW’s along with the on-site Grace team
• Order DME and equipment at the direction of the Grace RN
• Other duties as assigned within the persons scope of practice by the Grace Hospital Liaisons
• Ensure that Grace Healthcare Services’ Best Practices are incorporated into all job activities, and responsibilities.

Qualifications:

• College degree in the healthcare field is a plus
• Must have healthcare experience
• Excellent customer service and communication skills
• Ability to multitask
• Organizational skills
