Minimum Qualifications
Education: Bachelor of Science in Nursing required. Master’s degree in nursing or relevant field such as an MBA or MHA preferred. Candidates with a national certification in case management field will be given preference.
Experience : Two or more years of nursing related care experience and/or home care experience combined. Experience may include case management/discharge planning in one of the following settings: Acute care, Home Care, Long-term care, Physician Office, or a Managed Care company.
Licensure/Certification: Must be currently licensed as a Registered Nurse in the state of South Carolina or holds a current compact/multi-state license as a Registered Nurse in a recognized NCSBN Compact State and is not a permanent resident of SC. Must provide copy of national certifications if applicable. Must have a valid Driver’s License and good driving record. Must have a current American Heart Association BLS for Healthcare Provider Card.

Knowledge/Skills: Adheres to the CMSA Standards of Practice for Case Management. Knowledge of care management concept along the continuum. Knowledge of Medicare benefits. Ability to successfully articulate the process of attaining goals and outcomes of care management. Ability to apply clinical knowledge in a care management role. Understand and complies with regulations and recommendations of outside regulatory bodies with regard to providing case management services. Excellent verbal and written communications skills. Ability to engage and collaboration with the patients and significant others in the care management process. Knowledge of HIPAA and the ability to apply during the care management process. Demonstrates ability to prioritize activities on a daily basis and adapt effectively to changing priorities. Ability to care mange diverse populations without applying one’s own personal values. Ability to work with minimal supervision within nursing scope of practice. Ability to think critically and analytically and work with minimal supervision. Clinical knowledge and ability to educate clients of all ages about the following core disease management issues including but not limited to Diabetes, CAD, Asthma, COPD, and renal disease required.
