Job Summary:
Boston Health Care for the Homeless Program (BHCP) is seeking a new Clinical Team Coordinator to lead a group of 7+ care coordinators and/or case managers on our Behavioral Health Community Partners Teams. The Clinical Team Coordinator will be responsible for connecting highly vulnerable and homeless individuals experiencing mental illness and/or substance use disorders with behavioral health, primary care, and social determinants of health supports.
Hours: Full-time; Monday-Friday, 8am-4:30pm

To succeed in this role, you’ll bring strong leadership skills, clinical skills, and have the confidence to collaborate within a cross sector team that is based in various locations across Boston, providing feedback regarding population health. You must have experience caring for underserved populations, navigating homeless/housing services and the health care system. Supervisory experience is strongly preferred.
