POSITION DUTIES
This position works with newly enrolled Medicaid clients to access and navigate the Medicaid system and identify providers. In addition, this position helps link Medicaid clients to necessary community resources. Follows up on non-compliant clients.
This position is eligible for hybrid telework.
55% This position initiates contact with newly enrolled Medicaid clients (via phone, email or home visits) and assists with accessing and navigating the Medicaid system and identifying other support services in the community. This includes: assisting with identifying managed care organizations, assisting with identifying community providers (primary care and specialty providers), and linking clients to support resources in the community including food, dental, prevention (smoking cessation, etc.) mental health/substance abuse services, and more. This also includes educating clients on the appeal rights process. Refers complex cases to ACC nurses.
25% Conducts follow up on Managed Care Organizations (MCO) referrals of non-compliant clients by phone, mail, email and/or home visits. This includes Local Health Services Request Form Referrals (DHMH 4582).
15% Attends regular staff meetings, outreach events, trainings and provides additional programmatic support to the program as identified by the ACC Nurses, Ombudsman and or Program Director. Decrease client health disparities by utilizing CAREAPP.
5% Employee may be designed as an emergency essential employee at the time of an emergency. Responds to call down drills or actual emergency events as soon as possible, regardless of when message is received.
Minimum Qualifications
Education : Graduation from an accredited high school or possession of a high school equivalency certificate.
Experience : One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.
