Responsibilities:
– The Intake Coordinator will be responsible for managing the intake process for patients, including scheduling appointments, obtaining medical records, and coordinating with other departments.
– Answer incoming calls and provide information about available services
– Triage incoming calls related to patients, families, physicians, and staff Initiate referral process as outlined in department specific procedure
– Collect and enter patient information into the system accurately and efficiently
– Verify insurance coverage and obtain necessary authorizations
– Input new patients, new vendors, and new field staff into software system
– Provide patient education on hospice and palliative care services
– Schedule appointments and coordinate with medical staff – Maintain confidentiality of patient information and adhere to HIPAA regulations
– Assist with administrative tasks as needed
– Check the monthly schedule weekly and update on the go changes.
Qualifications:
– LVN LICENCE preferred
– BILINGUAL preferred – Previous experience working in a Hospice Agency.
– Excellent communication skills, both verbal and written
– Strong attention to detail and ability to multitask effectively
– Proficient in computer skills, including data entry and electronic medical records systems
– Ability to work well under pressure and remain calm in stressful situations
– Knowledge of medical terminology is a plus
This position requires strong organizational skills, attention to detail, and the ability to work well in a fast-paced environment. Strong communication skills are essential for effectively interacting with patients over the phone. The successful candidate will also possess excellent computer skills, including proficiency in data entry and electronic medical records systems.
