Part of the On-call manager rotation:
Job Description:

• Wound Care Coordinator is responsible for the overall management of the skin/wound care program.
• Addresses skin tears, rashes, pressure areas, changes in skin condition, follows up with new orders, treatment plans assessments. Change plan of care as needed, communicate changes/recommendations to attending physician, staff, and families.
• Completes weekly wound rounds with physician and documents resident treatment plans, Completes the admission assessment, MDS data as it pertains to the program, updates care plans with each MDS assessment as needed.
• Wound Care Nuse attends weekly resident care quality and QAA meetings.
• Nurse Manager on call duties as assigned
• Incident/Accident in regards to skin alterations with a follow up investigation and as needed.

Requirements:

• Must have RN license in good standing in Illinois, will consider LPN with Wound Care experience.
• Minimum 3 years industry experience
• Leadership and managerial experience.
• Must have excellent nurse assessment skills
• Must possess excellent computer and communication skills
