*Remote and In-Office Hybrid Role*
The role of referral coordinator is an important member of the health care team and is expected to demonstrate a high level of engagement with their co-workers, providers, and patients/families. Additionally, some critical thinking skills are necessary in order to follow standing orders and algorithms for screening, prevention, and care planning.
Responsibilities (included, but not limited to)
Education and Experience Requirements
Work hours: Full-time, hourly position. In order to maintain full-time status, a minimum of 30 hours per week on average are required, though 40 hours a week will typically be expected for this position. Hours are subject to change. Clinic hours are 8:00am to 6:00pm Monday through Friday and 8am -4pm on Saturdays. Referral Coordinator may be scheduled to work any of the days the clinic is open.
