Intake Coordinator
Community HealthCare is seeking a full-time Intake Coordinator to join our team. This person will be responsible for answering all intake questions and concerns, scheduling, and record keeping. This position will also be responsible for providing support to the Intake Team and assisting with patient care as needed.
Duties and Responsibilities
·Monitor all intake calls and schedule patients accordingly
·Schedule patients for testing and follow-up appointments
·Collect and record patient information in the electronic medical record
·Answer all questions from patients about the clinic, test results, or testing procedures
·Schedule follow-up appointments
·Communicate with patients regarding their results via phone or e-mail
·Maintain accurate records of scheduling and contact information for all patients
·Process all inbound intake calls and schedule new patients as needed
·Monitor intake forms and faxes for accuracy and completeness ensuring that all information is complete, accurate, and accurate. Coordinating with Intake Team to ensure accuracy of intake forms.
·Assist with patient care as needed including ordering supplies, calling in prescriptions, scheduling labs, etc.
·Maintain a clean work environment including trash removal throughout the shift. Cleaning floors throughout shift.
·Provide support to the Intake Team including providing feedback on practices, policies, or procedures as needed. Provide feedback to the Intake Team regarding any issues that may arise within the organization. Assist with follow-up with patients after care services are completed.
·Other duties as assigned.
