Guardian Care – Care Coordinator

We are hiring a Care Coordinator to genuinely make a life-changing impact in the communities we serve, who will be the primary onboarding contact for our clients and caregivers.

What are we looking for?
· Excellent Customer Service skills, including managing high call volumes with composure and positivity
· Strong written and verbal communication skills, and ability to solve problems with our clients and caregivers
· Exceptional attention to detail and organization skills – to meet compliance and governance standards from onboarding through to day-to-day operations.
· Tenacity and time-management skills – ability to take ownership, to be a self-starter committed to approaching administrative tasks with tenacity.
· Excellent work ethic in maintaining and enhancing Care Team and client morale.
· Adaptation to change quickly and with a positive attitude
· Familiarity with Office 365 (Excel, PowerPoint, Word, etc), Customer Relationship Management (CRM) tools, and Home Care software solutions
What are the key responsibilities of the role?
· Connect and build excellent relationships with new clients, caregivers, Case Managers and referral sources.
· Complete onboarding process for new caregivers, including building schedules, completing onboarding paperwork, setting up profiles within our ecosystem of tools, and completing background checks
· Live out Guardian Care’s Values: Treat Everyone Like Family, Integrity, Excel at Communications, Address Issues and Improve.
· Complete onboarding process for new clients, focusing on personalized Service Plans based on the needs, interests and preferences of clients
· Confirm client payor eligibility on a regular basis
