JOB FUNCTION:

Assist in the implementation of and/or providing additional information for the resident service plan and updating them on an ongoing basis. Communicate with physicians/other health providers regarding the resident’s health status when appropriate. Communicate pertinent resident information to co-workers when leaving the community and/or at shift changes. Assist care providers with the implementation of service plans. Determine when a resident requires room service for meals due to illness. Maintain accurate and complete resident file documentation. Responsible for effective overall coordination with other ancillary health providers to provide quality care and services to all residents.

Job Type: Part-time

Pay: $28.00 – $32.00 per hour
