The Wound Care Nurse Specialist will provide comprehensive wound management and ostomy nursing care to patients in collaboration with other members of the health care delivery team, including providers, staff nurses and wound care center staff. The wound care of patients includes, inpatients and those residing in the health systems Skilled Nursing Facilities or Long Term Care Serves. Serve as clinical expert, educator and liaison between the patient and the medical, nursing and interdisciplinary staff with the goal of wound healing and improved patient outcomes. In the execution of job duties, it is the universal expectation that all tasks are performed with a patient centered focus, while also seeking opportunities to continually improve core processes. Incumbent will be scheduled based on operational need, which may include but is not limited to: holidays, extended shifts, night and/or weekend shifts, standby and/or on-call. Required Skills Certifications:
Basic Life Support (BLS)
Wound Care Nurse Certification required within six months of hire.
Licenses: Current Virginia State RN License
Job Description
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Minimum Work Experience Required: Minimum of 2 years experience as in wound care management. ESSENTIAL FUNCTIONS Participates in direct patient care in relation to wound management as ordered by the medical provider. Implements patient care plans and teachings in collaboration with the patient, medical provider and the health care team. Demonstrates knowledge and skill in assessment and performing local wound care including: debridement techniques, cleansing, dressings, pressure relief, pain control and odor control in nursing facility. Evaluates and recommends Durable Medical Equipment to the provider for patients with debility and rehabilitation needs. Ensures necessary wound care products are available and works with vendors to maintain would care formulary. Supports the continuum of care as applicable to ensure appropriate equipment and services are available upon discharge. Responds promptly to consultation requests. Assesses, stages and measures wounds accurately and in accordance with policy utilizing current infection control techniques, wound care supplies and treatment protocols. Maintains accurate and timely documentation of patient encounters in electronic medical record. Utilizes analytical ability and clinical knowledge to develop treatment protocols and manage wound care and impaired skin integrity programs, policies and goals. Serves as an educational resource and provides in-services related to skin and wound care for the entire health system including medical, nursing and administrative staff Evaluates and documents patients’ response to wound care; monitors and recognizes changes in physiological and psychosocial status; takes appropriate action based on assessment of the patient.
