The Role:
As an Admissions RN, you will act as a vital bridge between referral sources, patients and their families, and Willamette Vital Health clinical staff. The comfort, compassion, teaching, and empowerment you provide can help hospice patients and their families make the best decision for their final months to maximize their quality of life. We are looking for nurses that want to use their passion and expertise as part of our vital team to provide the best care possible for hospice patients.
This position requires the following weekly schedule: Monday thru Friday, 8:00 am to 5:00 pm

By providing expert, personalized care to hospice patients where they live you will bring dignity and compassion to those coping with symptoms of a serious illness.

Requirements:
· Current Oregon RN license
· Bachelor of Science in Nursing and certification as Hospice and Palliative Care Nurse preferred
Minimum of two years of recent clinical experience as RN. We prefer experience in hospice and/or home health
· Must be vaccinated for COVID 19
· Current Oregon driver’s license, dependable transportation, and proof of current automobile insurance
· Current CPR certification
· Possess excellent clinical nursing skills, superior ability to problem-solve, and commitment to continued learning and skill improvement
· Ability to excel in high-stress situations and manage multiple priorities
· Computer competency and electronic medical record experience a must.
· Ability to regularly walk bend, stoop, kneel, push, pull, grasp, reach, handle, and lift up to 50 lbs.
Responsibilities:
· Coordinate the admission of patients to WVH programs by working with facility, hospital, transport, patients, families and WVH staff
· Use your teaching skills to empower family caregivers as you explain hospice services and WVH programs to help them identify their options for care to optimize quality of life
· Fully use your professional nursing skills and training, including critical thinking, teaching, and assessment skills as you provide admission services to patients, families and caregivers in their own homes, the hospital, and facilities
· Provide caring, professional nursing services, working as part of a team to meet the holistic needs of the patient and their family/caregivers
· Provide timely documentation and communication of discharge plan
· Act as a liaison between community referral sources, patients/family members, and WVH staff
