AmeriPro EMS Hiring for Patient Discharge Discharge Navigator Full Time

We are looking for a Patient Discharge Navigator, to join the AmeriPro Health team. In this role, you will support our efforts in the Emory Health system to ensure a comfortable and safe place for discharged patients to stay while waiting for transport home, or another health care facility. This position is responsible for coordinating the maintenance of a safe, clean and functional environment, facilitating communication, making discharge phone calls, assisting patients and directing visitors. As an integral part of the team, this position is responsible for the total patient experience while in the Patient Discharge Center.

Sign On Bonus: Full-Time Paramedic $5000, Full-Time EMT $2500

What you’ll do:

• Maintain a clean and orderly discharge center environment
• Greet and assist discharged patients, ensuring their comfort & safety
• Assist patients with self-administered medications and self-care during stay in center
• Recognize signs of patient deterioration and immediately activate rapid response team as appropriate
• Aid patients that are bed bound and ambulatory as needed during stay in the center
• Document patient time of pick up, source of transportation and other pertinent information
• Communicate with hospital staff regarding discharge center census

• EMS industry experience/background
• Excellent communication skills
• Understand and comply with confidentiality requirements
• Professional and friendly demeanor, and ability to work effectively in a team environment
• Demonstrated punctuality and dependability
• Self-motivated and able to help without constant direct supervision
• Ability to assess when guidance and direction is required

Requirements:

• High School Diploma or GED required
• Georgia State Certified EMT-B or Higher
• BLS Healthcare Provider CPR
• Current State Driver’s License

Job Type: Full-time

Pay: $16.00 – $19.00 per hour
