Pulmonary Clinics of Southern Michigan, PC is a private practice founded in 1981.

We are seeking a full time Physician Assistant or Nurse Practitioner to join our team!

This position will be covering the Jackson Michigan area providing pulmonary care for our patients in our office and the local hospital.

The ideal candidate will have clinical experience in any of the following areas: family practice, internal medicine, pulmonary medicine, rehab medicine, neurology, cardiology, emergency or geriatric medicine. Candidate must be able to practice independently with strong clinical decision making; be able to communicate clearly with patients and caregivers and be able to navigate well through the EMR. The best candidate will also possess a respectful, kind and mature demeanor.

Physician Assistant or Nurse Practitioner shall:

• Provide medical care for patients in our office and local hospital.
• Perform a complete history and physical on new patients.
• Assess the status of chronic conditions and acute issues at each visit.
• Complete a review of systems at each visit.
• Create a problem list or diagnosis list (assessment) and update at each visit if needed.
• Formulate a treatment plan based the H&P, Assessment and Problem List.
• Prescribe appropriate medications and evaluate their effects in terms of efficacy & safety
• Review and reconcile medications at each visit.
• Order and interpret indicated test results.
• Manage patient disease states.
• Implement health maintenance & preventative health measures.
• Educate caregivers and patients at each visit.
• Communicate with patients, caregivers and other healthcare providers in compliance with all applicable privacy laws and policies.
• Meet with the patients family to discuss medical care and end of life issues as indicated.
• Communicate with physicians as outlined in the practice agreement.
• Participate in case conferences with physicians and mid-level providers as needed.
• Document all information noted above in Epic EMR in a clear and concise manner.
• Clinically supervise ancillary staff including LPNs, medical assistants and other nurses and therapists.
• Assist in the oversight of homecare nursing and therapy.
• Practice medicine within the scope of licensure/certification/ and education within a office or hospital setting and according to a practice agreement.
• Be available to work weekends and nights if required.
• Demonstrate competence using the Electronic Medical Record (EMR) system Epic.

Administrative duties:

o Check email each business day.

o Check EMR messages each business day.

o Complete visit notes in a timely manner.

o Oversee nursing and therapy as indicated

o Maintain a thorough and complete patient chart.

o Participate in CMS Quality programs as directed by practice manager or physicians.
