CareCorp Seniors Services, a trusted long-term care and assisted living contractor, is currently hiring a Care Manager for one of our service delivery sites.

Position Summary

ing to the VP of operations, or designate, the Care Manager is responsible for providing guidance, supervision, leadership, coaching and the imposition of progressive discipline to all designated care staff employees. The Care Manager ensures that health codes, safety measures, accreditation, and legal mandates are met. The care manager collaborates with the client’s site management team, interdisciplinary team and stakeholders to ensure delivery of safe care and services.

Responsibilities

• Oversees the provision of clinical care and provides supervision, leadership and guidance to designated staff and team professionals to achieve excellence in the care of residents and families
• Works in collaboration with the client’s site management team, interdisciplinary team, residents/ families and stakeholders to identify and alleviate barriers to effective provision of care.
• Coordinates work activities to ensure an efficient and high standard of care and effective utilization of resources to meet operational needs.
• Provides support and supervision to designated staff and team professionals including employing effective communication strategies, conflict resolution, performance recognition, performance management and safe work practices.
• Works collaboratively with staffing coordinators, or designates, to identify and resolve day-to-day staffing issues to meet staffing requirements.
• In collaboration with the HR department, ensures compliance with the Collective Agreement, attends and participates in labor management meetings, investigates and resolves formal grievances.
• Investigates incident and accident s. Ensures adherence to policies and procedures as they pertain to WorkSafe BC regulations and all other health and safety codes.
• Works in collaboration with the VP of Clinical Operations, the client’s site management team and interdisciplinary team members to identify education needs and participate in the delivery of staff education.
• Oversees the timely completion and submission of s, including RAI-MDS assessment, investigative s and audits.
• Collaborates with interdisciplinary team members, the client’s site management team and stakeholders to ensure delivery of evidence-based, quality care and service to all clients.
• Participates in quality improvement initiatives and strategies to promote excellence in care and service delivery.
• Maintains all required documentation and records.

Qualifications

• Current RN or LPN registration with the British Columbia College of Nurses & Midwives (BCCN&M)
• Five (5) years’ recent clinical experience working with older adults and in residential care environment.
• One (1) year of progressive leadership experience in a supervisory and/or administrative capacity in residential care setting or an equivalent combination of education, training and experience.
• Cultural education and experience are preferred

While the SSO is in place, you will be limited to working with only one employer and at only one (1) private long-term care hospital (facility), licensed long-term care facility, registered assisted living facility, stand-alone extended care hospital or provincial mental health facility. Unless regional clustering applies. Regional clustering allows you to, within the same region and in agreement with the facility, hold a position as permanent with one (1) employer and hold a casual position with one (1) additional employer. You may hold only one (1) permanent position, one (1) additional casual position and must not work outside of your regional cluster.
