Job Summary

Provides a service navigation role for Child & Adolescent Mental Health Service- Child and Family Clinic. The Intake Worker triages all referrals, responds to all inquiries and processes referrals for smooth admission or redirection to the appropriate program. Acts as a resource to the community regarding service options and fields all inquiries regarding children, youth with mental health challenges, with the goal of streamlining access to Child and Family Clinic at Southlake Regional Health Centre. Records, manages and reports on all referral and Intake activity in the department, addresses gridlock and impediments in the system, and works effectively with both external and internal stakeholders. Provides brief interventions to families as needed.

Qualifications

• Advanced diploma or equivalent degree in a social science, preferably in the area of children’s mental health or Bachelor of Child & Youth Work.
• Minimum of 3 years current and relevant professional clinical experience in mental health and eating disorders services for children and families.
• Experience in Intake is an asset
• Knowledge of (or willingness to learn) about York Region and Simcoe County community services, service capacities, and referral processes.
• Knowledge of data compilation, analysis and reporting.
• Demonstrated excellence in clinical practice; must be experienced in brief treatment modalities Demonstrated collaborative working relationships with inter-professional team members, and strengths in interpersonal communication, diplomacy, coaching, and facilitation.
• Demonstrated commitment to a family-centred / systemic treatment orientation.
• Demonstrated knowledge and understanding of hospital information systems.
• Demonstrated knowledge of software applications, including the ability to conduct telephone conversations and key in data simultaneously.
• Excellent ability to maintain, monitor and report on all data relevant to Intake processes for the department.
• Excellent written and verbal communication skills.
• Demonstrated skill in critical / analytical thinking, organizational, problem solving and decision-making skills.
• Demonstrated ability to work both independently and collaboratively as a team member.
• Demonstrated initiative.
• Demonstrated ability to work in stressful situations and be very flexible.
• Demonstrated ability to represent the department and work in partnership with external stakeholders (referrers, new client referrals and their families, departmental programs and community agencies), as well as participate in community initiatives impacting Intake and referral processes.
