Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Family Medicine Physician in Willow Creek, California. Requirements
- MD or DO REQUIRED
- Board Certified or Board Eligible in Family Medicine REQUIRED
- Active California medical license or ability to obtain REQUIRED
- Commitment to mission-driven underserved care REQUIRED
- Comfortable practicing within collaborative outpatient care environment REQUIRED
- Strong communication and patient relationship skills REQUIRED
- New graduates welcome
- Rural health interest strongly preferred
Responsibilities
- Provide outpatient-only primary care services to diverse patient populations
- Manage acute and chronic primary care conditions within collaborative care model
- Partner with multidisciplinary support teams including 2 MAs and RN support per provider
- Utilize integrated services including behavioral health, telepsychiatry, chiropractic, acupuncture, and specialty consult resources
- Participate in quality improvement initiatives and team-based care meetings
- Deliver compassionate, mission-focused care to underserved rural communities

- Base salary up to $267,624 annually

- Partnership eligible sign-on structure: $50,000 sign-on bonus, $25,000 retention bonus in year 2, $25,000 retention bonus in year 3
- Non-partnership sign-on structure available
- $10,000 relocation reimbursement
- Additional $10,000 relocation assistance for parent or in-law relocation
